By entering Champions Adaptive Fitness premises or participating in any program, class, or event, you acknowledge and agree to the following:

1. Security & Safety Recording

• Our facility uses continuous video surveillance for safety, training quality, and security.

• These recordings may capture participants during classes, training sessions, or general facility use.

• Security footage is not used publicly unless separate consent is given.

2. Promotional Media Consent

You grant Champions Adaptive Fitness permission to photograph, record, or film you or your enrolled minor during sessions, classes, or events for the purpose of:

• Marketing

• Social media content

• Website materials

• Educational or promotional use

This includes edited or unedited versions across any media platform.

3. Rights & Waiver

You understand and agree:

• You will not be compensated for the use of your image, voice, or likeness.

• Champions Adaptive Fitness owns all rights to the produced content.

• You waive any right to inspect or approve the final media.

• You release Champions Adaptive Fitness from any claims related to the use of such media.

4. Opt-Out Option

You may request to opt out of promotional recording at any time by notifying staff in writing.

Note: This does not apply to security camera footage, which cannot be disabled.
