Please read carefully. By participating in any Champions Adaptive Fitness program, you acknowledge and agree to the following:

1. Assumption of Inherent Risk

I understand that fitness activities—including adaptive training, sports performance, strength training, stretching, mobility work, and use of gym equipment—carry inherent risks, including but not limited to:

• Falls

• Collisions

• Muscle strains or injuries

• Medical events

• Accidents caused by equipment or other participants

I voluntarily accept all risks associated with participation for myself or my enrolled minor.

2. Medical Responsibility

I certify that I or my enrolled minor:

• Am physically and medically able to participate

• Have disclosed any medical conditions to staff

• Understand that staff are not medical professionals

I agree to seek medical clearance when needed.

3. Release of Liability

To the fullest extent permitted by law, I release and hold harmless:

Champions Adaptive Fitness, its owners, staff, trainers, contractors, and affiliates

from any and all liability for:

• Personal injury

• Illness

• Property damage

• Emotional distress

• Death

  arising from participation in fitness activities, except in cases of gross negligence or intentional misconduct.

4. Indemnification

I agree to indemnify and defend Champions Adaptive Fitness from any claims brought by or on behalf of the participant.

5. Emergency Consent

I authorize staff to obtain medical care for me or my minor in emergencies, and I accept responsibility for associated costs.

6. Acknowledgment

By using our facility, registering online, or participating in any session, you confirm that you:

• Have read this agreement

• Understand it

• Agree to its terms voluntarily
